Methods of Payment (see also FAQs)

  • Tricare Military Insurance now accepted

  • Cash or Check (Preferred)

  • Credit Card: Visa, MasterCard, HSA/FSA cards may be used for payment

  • Med-Pay: Through your Auto Insurance coverage

  • Medicare:  I cannot see any Medicare patients for services that might otherwise be covered by Medicare.  I can cover any wellness-based services. Medicare cannot be charged and you cannot be reimbursed by Medicare.

  • I am an ‘Out-of-Network’ Provider

    • I do, however, accept Tricare

    • For integrative physical therapy consult and treatment,  a superbill/invoice will be provided to you, which you can submit to your insurance company for reimbursement for ‘Out-Of-Network’ services.

    • Here is a link to an example  FORM that you can easily find on YOUR health insurance website or call your insurance company to have them send you one. You will send my superbill/invoice, your doctors prescription for PT (if required by your insurance company) and the claim form to your insurance company.  This applies only for PPO plans and does not cover HMO or Medicare. Most clients are able to get reimbursed for a percentage of their investment after any deductible is taken care of. It is your responsibility to follow-up with your insurance provider, but I am happy to assist you with the process.
